The extension agreement shall include the complete amount past due. Extension agreements are available to customers only once every three months for a maximum of four times in a twelve month period, may not be consecutive. Failure to comply with the terms of any extension agreement will result in disconnection of service without further notice; customer will not be eligible for any additional extensions for twelve months following a broken extension; and a $27.50 reconnect service fee imposed.
