Request a certified copy of a North Dakota born birth certificate online. A certified copy of a birth record may only be requested by the individual named on the record, provided they are at least 16 years old or the mother or father named on the record.

We must also require proof of identification of the person making the request before we can release a certified copy of a birth record.  Parents can submit their own ID when requesting their children's birth record.  Proof of identification can be established by submitting a current (non-expired), legible photocopy of one of the following forms of identification: 

• State Government issued Photo ID or Driver's License
• Bureau of Indian Affairs issued tribal ID card
• US Government issued Military ID card
• US Government issued Passport or Visa
• US Government issued Permanent Resident Card
